Waiver, Assumption of Risk and Agreement to Hold Harmless

I/we fully understand and appreciate the potential dangers, hazards, and/or risks, directly and/or indirectly inherent in participating in this activity. I/we acknowledge that I/we understand the intent of the services provided by SUPERNOVA DOGS, LLC. I/we understand that attendance at any dog training session and/or behavior counseling session is not without risk to myself, members of my family, my dog, or any guests who may be present at a training session and/or behavior counseling session, because a few dogs to which I/we will be exposed may be difficult to control and could be a cause of injury. 

I/we represent that I/we are the legal owners of (name of dog declared above in the Registration section), that title to said dog is not mortgaged in any way, and that said dog has not been exposed to distemper, rabies, kennel cough, parvo, or other known contagious diseases within the last 30 days.

I/we attest that said dog is current on all commonly recommended vaccinations. I/we also attest that said dog is free of worms, ticks and fleas. I hereby waive and release SUPERNOVA DOGS, LLC, any class instructors, volunteers or agents from any and all liability of any nature, for injury or damage resulting from any event which transpires at any training session and/or behavior counseling session as a result of any action by any dog, including my own. I assume the risk of any such damage or injury while attending any training session and/or behavior counseling session, or while on the training grounds or the surrounding area. I/we expressly assume the risk and financial liability of any such damage or injury while attending and/or participating in any training session and/or behavior counseling session or other function.

I/we hereby waive and release SUPERNOVA DOGS, LLC, any class instructors, volunteers or agents from any and all actions, claims or demands that I/we, my/our heirs, guardians, administrators, executors, legal representatives or assigns now have or may in the future have for injury, loss, damage from disease, death, running away, injury to persons, injury form other dogs, to my/our dog resulting from my/our dog’s activities with SUPERNOVA DOGS, LLC, whether or not resulting from the negligence or misconduct of any person, or the actions of another animal. In the event that SUPERNOVA DOGS, LLC deems it necessary to employ legal counsel to protect the rights of herself or any class instructors, volunteers or agents under this agreement, the owner/agent of the dog agrees to pay all expenses incurred by SUPERNOVA DOGS, LLC to enforce their rights under this agreement including but not limited to costs and any reasonable attorney’s fees.

I/we understand the rate schedule and agree that all fees are due and payable at the time services are rendered, unless other agreements are made in writing with SUPERNOVA DOGS, LLC.

I/we THE UNDERSIGNED hereby agrees to abide by the rules and regulations of SUPERNOVA DOGS, LLC, any class instructors, volunteers or agents. I understand that the instructors have the right to refuse to instruct any aggressive dog at their discretion. Because behavior in dogs and people are unpredictable; we have a no refund policy.  

I/we have read, understand, and agree to all of the terms, conditions, and statements above. I am at least eighteen years of age. If not eighteen years of age, my/our parent/guardian has also read, understands, and agrees to the above. This contract is in effect beginning on the date below and shall remain in effect and is binding in perpetuity until written notice by certified mail is provided by the undersigned to SUPERNOVA DOGS, LLC.
